Did you know the Army Provider Level Satisfaction Survey, or APLSS, is one of the best ways to improve the quality of care you and your family receive at the Irwin Army Community Hospital?

The short survey tells the hospital how your experience went. And, your suggestions are taken seriously.

If we can improve our quality of care for you, our patients, we want to know.

By filling out your survey with positive comments, you’ll be helping your hospital get some much-needed increased funding.

That extra money will be used for things like better equipment and more training for our staff, so we can provide you and your family the very best in up-to-date health care.

We also want to know how we can improve your experience with us, so we appreciate those comments as well.

The APLSS survey takes about five minutes to fill out. Soldiers can receive the survey via email and can fill it out online, by telephone or by mail. Families can turn in the survey by mail.
